Provides assistance with daily activities that generally help people with disabilities to remain in their homes, such as assisting with activities of daily living (ADL's): bathing, dressing, using the toilet and instrumental activities of daily living (IADL's) such as shopping, managing money or medications and doing laundry. All services are in accordance with your loved ones' ISP.
Provides relief for the primary care givers in their absence in an emergency situation. This service is provided in the family's home.
Your loved one can stay in a familiar environment, maintain comfortable surroundings and routine. Your loved ones will receive help with all ADL's and IADL's such as meal preparation, escorting them to any needed appointments or other activities that are planned in the caregiver's absence. All services are in accordance with your loved ones' ISP.
Home and Community Habilitation
Provided to help people gain, maintain, and improve skills that allow them to live and participate in their local community. These services help your loved ones with acquiring, maintaining, and improving self-help skills. The service consists of support in the general areas of self-care, communication, fine and gross motor skills, mobility, socialization, and use of community resources. All services are in accordance with your loved ones' ISP.
